9-6 PULMONARY FUNCTION STUDY

PATIENT: Hag Ulrich

PHYSICIAN: Gregory Dawson, MD

ENTRANCE DIAGNOSIS: Dyspnea in a patient who has a 67.5-pack-year history of smoking and has a nonproductive cough. Gave good consistant effort.

9-8 SLEEP STUDY

LOCATION: Outpatient, Hospital

PATIENT: Charlie Grove

PHYSICIAN: Gregory Dawson, MD

STUDY PERFORMED: Nocturnal polysomnogram with CPAP (continuous positive airway pressure) titration.
